Krsnaa Diagnostics: Teleradiology at National Scale Across 331 Districts

Krsnaa Diagnostics is one of India's largest diagnostics providers and a leading operator of public-private-partnership (PPP) diagnostic services, delivering affordable radiology and pathology to patients largely within government hospital premises and district health facilities. The company operates more than 1,000 radiology centres and over 500 pathology centres, spread across 18 states and union territories and 331 district locations, and has served more than 81 million patients since inception.

Krsnaa's network footprint includes 190 CT/MRI centres, 1,434 telereporting centres, 147 pathology labs and more than 4,700 collection centres, with 54 NABH-accredited centres and accredited laboratories underpinning its quality programme. Its operational mix varies by geography: full-spectrum CT, MRI, X-ray and pathology operations in states such as Maharashtra and Punjab, CT and X-ray operations in states including Madhya Pradesh and Andhra Pradesh, MRI operations in Chandigarh and Karnataka, and pathology-led operations in states such as Rajasthan and Manipur, extending as far as Jammu & Kashmir and Himachal Pradesh in the north and the north-eastern states.

The Challenge

Krsnaa's business model creates an imaging challenge that is almost the inverse of a hospital enterprise. Instead of a small number of large, well-staffed facilities, Krsnaa operates a very large number of lean, distributed centres, many of them in tier-2, tier-3 and rural district locations where recruiting and retaining on-site radiologists is not feasible. Scan acquisition happens at the periphery, in over a thousand centres across the country, while reporting expertise must be concentrated and shared.

The platform supporting this model had to move very high daily study volumes reliably over variable public-sector network connectivity, route every study from any acquisition centre to available radiologists at central reporting hubs, enforce the strict turnaround-time commitments that PPP contracts with state governments demand, keep per-study costs extremely low to sustain affordable pricing, and scale continuously as Krsnaa won new state contracts and added districts, without re-architecture at each expansion.

The Medsynapse Solution

Medsynapse powers Krsnaa's radiology operation as a hub-and-spoke teleradiology PACS at national scale. Each radiology centre, whether a standalone X-ray unit inside a district hospital or a full CT/MRI facility, acts as a spoke: studies acquired on local modalities are transmitted to the central Medsynapse platform, where they are archived, quality-checked and placed onto unified reporting worklists. The company's 1,434 telereporting centres feed into centralised pools of radiologists who report through the Medsynapse web viewer and RIS, with structured worklist prioritisation ensuring that emergency and time-critical studies are read first.

Because the viewer is zero-footprint and bandwidth-efficient, reporting radiologists need only a browser and an internet connection, which allows Krsnaa to flexibly scale its reporting panel and cover nights, weekends and volume surges without geographic constraints. The RIS layer manages registration, order tracking, report generation and delivery back to the originating centre and the treating government facility, providing end-to-end visibility of every study from acquisition to signed report. Centralised administration lets a compact IT team onboard new centres rapidly, a critical capability given the pace at which the network has grown across 331 districts.

With Medsynapse as the connecting fabric, every Krsnaa centre, regardless of how remote, has access to the same central pool of reporting expertise. Patients in district locations receive radiologist-reported studies of the same standard as those in metropolitan centres, which is the core promise of the PPP diagnostics model. Centralised worklists and turnaround monitoring allow Krsnaa to meet contractual reporting commitments to state governments consistently and to demonstrate compliance with auditable data.

Operationally, the hub-and-spoke architecture keeps the cost per study low: peripheral centres carry minimal IT infrastructure, radiologist capacity is utilised efficiently across the whole network rather than being stranded at individual sites, and new centres can be brought online quickly as contracts expand. The centralised archive also gives Krsnaa a longitudinal imaging record for the tens of millions of patients it serves, supporting follow-up imaging comparisons, quality review and accreditation requirements across its NABH-accredited centres.

The result is one of the largest distributed radiology operations in the country, in which more than a thousand acquisition points function as a single coordinated imaging service, enabling Krsnaa to deliver affordable, high-quality diagnostics at a scale of more than 81 million patients served.
